On the first screen, you can only proceed if you are not insured under ESI.
On the second screen, enter your Aadhaar Number and your mobile number as in Aadhaar.
Check the box to agree to share your Aadhaar details with the Directorate of Cultural Affairs on purpose of performing a digital KYC.
Now on the third screen, you would get an OTP from UIDAI on your Aadhaar registered mobile number.
Enter your OTP for verifying your Aadhaar number.
Once verified, you will be prompted to enter a password for your account on the next step.
After you set your password, you will be logged in.
Please note your password down. You can use this password and a combination of Mobile No + Password or Aadhaar No + Password to login to the system.
Next on the logged in area, you will be provided with the application form.
Fill up this form with your details.
Some of the fields in this form are already pre-filled with data from your Aadhar KYC. You cannot change those.
Email Address & PAN number is optional.
Select your District & LAC correctly.
You can click on "Save" to save your current data.
Click next to go to file uploads.
You have to upload two files mandatorily. One is your profile photo and the other is a PDF file detailing of any cultural achievements or awards received by you.
Uploading a PAN card softcopy is optional.
Make sure, the file sizes for images is less than 500 KB and file size of the PDF is less than 1MB
You must go through your entire form before submitting it.
When you click on "Submit", you will be asked with a confirmation if you would like to submit it.
When you submit a form, you wont be able to edit it anymore.
But, if you save it, and return any other day, you can continue from where you have left it.
You will have to submit your form, before the last date of form submission as stated by the Directorate.
One Aadhar number once applied, cannot be used again to file another form.
